Spring Valley Health and Rehabilitation Center operates a 194-bed nursing facility at 2915 South Fremont Avenue in Springfield’s Greene County. The facility is administered by Holly Anderson under ownership by MC Fremont Healthcare, LLC. It accommodates Medicare, Medicaid, and private-pay residents and maintains an occupancy rate of 80%, notably above the Missouri state average of 66.6%.
Spring Valley carries a 2-star overall CMS rating. The health inspection rating is 1 star, placing the facility 65.5% below the Missouri average. Its deficiency record since 2018 totals 78 citations; 123% above the typical Missouri nursing home.
Inspections conducted in August and November 2025 identified medication documentation errors, tube feeding care deficiencies, and environmental sanitation failures.
