New York State Veterans Home at Montrose is a New York State Department of Health-operated 252-bed skilled nursing facility at 2090 Albany Post Road in Montrose, New York. It serves eligible veterans and their dependents, providing skilled nursing, memory care, rehabilitation, and respite services. Accepted are Medicare, Medicaid, and private pay.
CMS scores the facility 1 star, with health inspection and quality measure sub-ratings each more than 49 percent below New York’s average. Across three inspections since 2023, the facility averaged 13 deficiencies annually, 155 percent above the 5.1 state average. An April 2025 investigation cited failures in abuse prevention, reporting of alleged violations, and residents’ rights to be free from chemical and physical restraints; all deficiencies were corrected by May 2025. No fines or immediate jeopardy findings are in the record. Staffing runs counter to the overall rating: nurse hours at 4h 15m per resident per day are 22 percent above New York’s average, placing the facility 55th of 388 state SNFs. Occupancy stands at 80 percent, modestly below New York’s 88.3 percent average. The facility’s veterans-specific mission limits eligibility to qualifying veterans and dependents in Westchester County.
The state-operated community suits eligible veterans needing skilled nursing or memory care in Westchester County.
Operating since 2015, Cedar Manor Nursing and Rehabilitation Center maintains 153 beds at Cedar Lane in Ossining, Westchester County, New York. Cedar Manor Holding I LLC, a for-profit operator, staffs the facility 24 hours daily with nursing care averaging 3 hours 10 minutes per resident. Current occupancy stands at 92 percent. The community serves residents requiring respite care, rehabilitation services, and memory care, including dementia and Alzheimer’s disease programming.
The deficiency pattern over the past 30 months constitutes the facility’s defining issue. Since September 2022, state inspectors have documented 52 violations at a rate of 13 per year; triple the New York baseline of 5.1 annually. The April 2025 complaint investigation identified 30 deficiencies across a broad operational span: activities of daily living, antibiotic stewardship protocols, COVID-19 immunization documentation, dialysis services, waste disposal, unnecessary medication use, misappropriation prevention, mobility and range of motion, infection control, drug storage and labeling, resident rights, respiratory care protocols, staffing adequacy, cooking and food preparation, emergency egress, HVAC and mechanical systems, rubbish chute maintenance, sprinkler testing, corridor integrity, electrical systems, emergency lighting, hazardous area enclosure, egress illumination, door testing and maintenance, and organizational structure. Many corrections were documented by May 2025.
Earlier in 2024, a January investigation uncovered 17 similar Level 2 violations across care planning, pharmacologic review, fall prevention, infection control, regulatory compliance, resident notification, quality measures, resident autonomy, and infrastructure safety. Three recurring violations involving the failure to report to the national health safety database remained unaddressed across 2022 and 2023 surveys. A June 2024 complaint investigation substantiated allegations of abuse and neglect, both corrected within weeks. No fines, immediate jeopardy citations, or license restrictions appear in the public record.
Named programs encompass Complex Medical Care, intravenous therapy, Cardiac Rehabilitation, Pain Management protocols, Wound Management, and specialized Dementia and Alzheimer’s disease care. The facility is named defendant in 22 lawsuits filed since 2015; 2 active medical malpractice cases are currently pending in Westchester Supreme Court.
Before placing a loved one, families should review current infection control policies, abuse prevention and reporting mechanisms, medication management documentation practices, and written responses to deficiencies identified in 2024 and 2025 inspections.
