But staffing gets 1 star. Here’s why: nurses provide 2 hours 32 minutes per resident daily. New York’s average is 3 hours 29 minutes.

National baseline is roughly 3 hours 30 minutes. This facility ranks 374 out of 388 New York facilities for nursing hours. RN turnover sits at 45 percent; nurse aide hours fall 23 percent below state norms. Physical therapy runs 4 minutes per resident per day against a state average of 7 minutes; a 43 percent gap.

Regulatory record: 10 state inspections since 2019 produced 25 citations (92 percent above the state average). Recurring themes: how care is delivered, medication administration, how staff handle resident needs, building safety. The facility corrects deficiencies promptly.
