Cascade Terrace Post Acute is a 105-bed post-acute skilled nursing and memory care facility at 5601 SE 122nd Avenue, Portland, Multnomah County, Oregon, 8.3 miles from downtown. Operated by Cascade Terrace SNF Healthcare, LLC, the community provides 24-hour skilled nursing with rehabilitation, memory care, therapy and assessment services six to seven days weekly, case management, discharge planning, and specialized programs including joint replacement and orthopedic rehabilitation, stroke rehabilitation, cardiac rehabilitation with CHF protocol, fall prevention, pulmonary care, post-operative care, pain management, infusion therapy, vascular wound management, renal disease services, tracheostomy care, cancer recovery, bariatric care, LVAD support, and palliative and hospice care.

The regulatory record documents severe and persistent care quality and operational failures spanning medication management, staffing, abuse prevention, and compliance systems. Over two years, Cascade Terrace logged 61 deficiencies; 30.5 annually, 355 percent above Oregon’s 6.7-deficiency benchmark.

A January 31, 2024, complaint investigation and state licensure survey revealed twenty-one deficiencies spanning vaccine consent, reasonable accommodations, change notification, abuse prevention and injury investigation, care plan implementation and revision, elopement prevention, accident hazards, staffing sufficiency, nurse aide training, behavioral health services, medication error prevention, binding arbitration, infection control, and in-service training. Several immediate jeopardy situations were identified and abated during this inspection. An April 28, 2025, complaint investigation documented twenty-one deficiencies in medication self-administration assessment, privacy and confidentiality, comprehensive and timely assessments, accurate MDS coding, ADL care, activity programming, quality of care, accident hazards, dialysis, RN staffing, medication storage, dietary needs, infection control, and payroll-based journal reporting; many were corrected by follow-up but some persisted uncorrected.

A January 23, 2025, survey cited five deficiencies in ADL care, quality of care, and fall prevention care plan implementation with partial correction. A November 6, 2025, survey identified four deficiencies including failure to clarify chemotherapy medication orders resulting in adverse outcomes, with multiple citations uncorrected at visit. Complaint surveys on September 23, 2025, March 6, 2025, August 21, 2024, and March 12, 2024, noted citations uncorrected at visit. Two January 2024 focused surveys cited COVID-19 NHSN reporting failures.

The January 2024 immediate jeopardy findings, the April 2025 twenty-one deficiency cluster, and the November 2025 chemotherapy medication order failure resulting in patient harm, constitute a pattern of fundamental care and systems failure.

While some facility responsiveness to revisit audits was documented, the persistence of uncorrected deficiencies across complaint investigations from January 2024 through November 2025 indicates incomplete corrective implementation and sustained operational fragmentation. No fines or license actions are recorded.

The facility provides post-acute and skilled nursing care with rehabilitation, memory care, and specialized services. Private pay is required; Medicaid and Medicare are not accepted. Families should obtain the January 31, 2024 immediate jeopardy documentation and the April 28, 2025 twenty-one deficiency report, addressing medication management, staffing, care planning, and abuse prevention, and require on-site verification of corrective implementation.
