Substantiated failures around adequate care and supervision caused multiple resident falls and serious injury; the facility had no updated care plan and didn’t relocate the resident. Separately, substantiated failure to follow eviction procedures: missing the required 30-day written notice, appeal rights documentation, and discharge planning. Yet the most recent annual inspection in December 2025 found no deficiencies. Clean, sanitary, in good repair, met all standards.

Operated by Pacifica El Norte LP (Escondido El Norte Mgr LLC), administered by Monica Flores; Julie Walke is the associate. Licensed as residential care for the elderly. Four years in operation, with occupancy at 72% (73 of 101 beds), in line with the California average.
It’s a three-storey building with studio and alcove units. Walk Score 69 (moderately walkable). There is no admission fee.
Respite care and 24-hour staffing are provided. Specialized dementia care is available, with live-in staff and separate housekeeping.

Yes, nine total citations (10% below CA average) and zero serious citations rank the facility #1 in California. But nine moderate citations is 125% above the state average; that’s roughly triple the expected level. Total visits were 25 (56% above CA average), complaint visits 15 (15% above), and complaint investigations 12 (20% above). The facility faced sustained regulatory attention.
The reason becomes clear in the March 2025 findings. Complaint investigations substantiated deficiencies for failure to provide toilet paper to two residents, medication errors for one resident, unsafe chemical storage, and staffing shortages causing long resident wait times.
These are not administrative violations or minor gaps; these are failures at the level of basic care provision and resident safety. The facility was understaffed enough that residents couldn’t access basic hygiene supplies and experienced dangerous wait times. Medication errors suggest care coordination failures. Chemical storage was unsafe enough to be cited.
A March 2025 complaint added another layer: failure to provide transportation services as promised, with the facility imposing a transportation policy that contradicted the admission agreement. That’s a financial and service consistency failure.
Then in February 2026, the facility passed inspection with full compliance and no deficiencies in physical plant, medication management, food service, care, or records.
Gardens at Escondido documents a severe operational failure in March 2025 involving basic care deprivation, medication errors, unsafe conditions, and staffing inadequacy, followed by rapid correction to compliance status in February 2026. The question is whether this represents genuine systemic remediation or temporary improvement while under scrutiny. The gap between “residents couldn’t access toilet paper” and “full compliance” in eleven months is substantial.
