Over 3.5 years, the facility averaged 2 deficiencies per year, 61 percent better than New York’s average. Six complaints are on record (7.1 per 100 occupied beds versus state average 49.9). No enforcement actions have been taken.

The September 2024 deficiency pattern shows problems across five critical areas simultaneously, suggesting systemic rather than isolated issues. The rapid remediation within months and subsequent clean inspection provide evidence of corrective action, but the breadth of the original violations warrants direct verification during tours of current procedures and facility condition.
