While this is 42 percent fewer than Oregon’s average of 31 deficiencies per facility, per-inspection deficiency counts are markedly elevated: Laurel Parc averaged 9 deficiencies per inspection versus Oregon’s state average of 4.4; 105 percent worse. Substantive deficiency themes included failure to monitor changes in resident condition, incomplete RN assessments, medication errors, inadequate restraint assessments, insufficient staff training, and fire and life safety instruction gaps. Kitchen cleanliness, infection prevention, and licensing rule compliance were also cited. Some deficiencies were corrected on revisits; others persisted initially. No fines, immediate jeopardy findings, enforcement actions, or substantiated complaints were reported. The facility has not undergone inspection since March 2023, leaving a three-year gap in regulatory oversight.

The most recent inspection on July 31, 2025 identified 1 deficiency related to kitchen practices and food sanitation rules. May 21, 2024 found 4 deficiencies including kitchen cleanliness, food sanitation, administration compliance, and inspection intervals. December 6, 2022 identified 12 deficiencies including failure to provide adequate activities, resident evaluations, service plans, health services, fire and life safety instruction, and staff training; November 29, 2022 found 4 deficiencies (kitchen cleanliness, food sanitation, inspection intervals, administration compliance). Some deficiencies were corrected on revisits; others persisted. Eleven licensing violations were recorded. No fines, immediate jeopardy findings, or enforcement actions were reported. Complaint investigations were not noted.
