One resident eviction lacked a signed 30-day notice and another lacked documented ombudsman notification. The facility’s washing machine and clothes dryer were unavailable for resident use.

No complaint investigations or prior inspection reports were available for historical comparison.

The facility structures itself around private-pay memory care and assisted living in a rural Utah environment, though the May 2024 inspection documented wide-ranging administrative, personnel, and emergency-readiness deficiencies requiring correction.
