Mealtimes are always nourishing with fresh and delicious meals served three times a day with accommodations for special diets. At Brookhaven Health Care Facility, residents are provided with all the amenities and services they need to maintain their maximum potential and live fully.
Located at 391 North Country Road in Smithtown, New York, Smithtown Center for Rehabilitation & Nursing Care operates 162 licensed beds under private ownership by Smithtown Health Care Management. Current occupancy stands at 99 percent, substantially above the state average of 88.3 percent.
The facility provides respite care, rehabilitation services, and memory care. Therapeutic Recreation Services represent the documented program offering. Nursing care runs 2 hours 32 minutes per resident per day. The facility does not maintain 24-hour staffing and does not accept Medicare or Medicaid payment.
The regulatory record documents persistent compliance difficulties. Over a four-year span, 31 deficiencies were cited at an annual rate of 7.8, significantly above New York’s 5.1 average.

Two December 2022 COVID-19 surveys identified national health safety network reporting deficiencies marked as widespread in scope, neither of which had been resolved at the time of reporting.
No monetary sanctions, immediate jeopardy determinations, or license suspensions are listed.
Legal exposure includes four currently pending cases among 16 matters filed since 2013.
