The most recent inspection on April 22, 2025, cited one deficiency for delayed hospital transfer and inadequate assessment after a resident’s fall causing fractures and surgery. Earlier investigations found immediate jeopardy from elopement due to poor supervision in February 2025 and multiple deficiencies in infection control, medication errors, and resident dignity during an August 2024 routine survey.
