Operated by Creston SNF Healthcare, LLC, the community provides 24-hour skilled nursing with rehabilitation, memory care, therapy and assessment services six to seven days weekly, case management, discharge planning, and specialized programs including joint replacement and orthopedic rehabilitation, stroke rehabilitation, cardiac rehabilitation, fall prevention, pulmonary care, post-operative care, pain management, infusion therapy, vascular wound management, renal disease services, tracheostomy care, cancer recovery, bariatric care, LVAD support, and palliative and hospice care.

The regulatory record reflects steady improvement with clean recent compliance. Over five years, Reedwood logged 30 deficiencies, averaging 6 annually, 10 percent better than Oregon’s 6.7-deficiency benchmark.

A December 10, 2021, re-licensure survey cited four deficiencies in food safety, activities, and administrative rules; some were corrected on some visits, many remained uncorrected. Four focused infection control surveys from December 2021 through December 2022 each identified one deficiency in failure to report complete COVID-19 information to CDC’s NHSN. A December 5, 2022, re-licensure survey documented eight deficiencies in care plan timing, activities, lab services, food safety, infection control, and administrative compliance; some corrected on first visit, others not. A February 21, 2023, focused infection control survey identified one COVID-19 NHSN reporting deficiency.

A March 7, 2024, complaint investigation revealed nine deficiencies in facility environment, abuse prevention, bed hold policy notification, assessment accuracy, ADL care, and activities programming; some corrected on first visit, others not. Complaint investigations on June 5, 2025, and June 26, 2025, found no deficiencies. The five-year arc shows resolution of recurring administrative and operational issues, particularly in food safety and activities programming, with COVID-19 reporting gaps addressed and environmental and care quality concerns from 2024 resolved by mid-2025. Recent zero-deficiency surveys align with the facility’s below-state-average overall deficiency rate.
