Complaint investigations on June 5, 2025, and June 26, 2025, found no deficiencies. The five-year arc shows resolution of recurring administrative and operational issues, particularly in food safety and activities programming, with COVID-19 reporting gaps addressed and environmental and care quality concerns from 2024 resolved by mid-2025. Recent zero-deficiency surveys align with the facility’s below-state-average overall deficiency rate.
