King David Center

2266 Cropsey Ave, Brooklyn, NY 11214
Overview of King David Center

King David Center for Nursing and Rehabilitation operates 271 beds in a high-rise building at 2266 Cropsey Avenue in Brooklyn. Operated as a voluntary nonprofit by Arthur New, the facility serves a balanced mix of Medicare (50% of admissions), private-pay (49%), and Medicaid (1%) residents, with current occupancy at 98% and average stays of 193 days. Administrator David Schoenblum leads operations with a clinical team spanning nursing, therapy, social work, dietary, and maintenance.

Specialized care offerings span skilled nursing, memory care, and rehabilitation. The facility operates cardiac rehabilitation, orthopedic rehabilitation, neurological rehabilitation, robotics and technology programs, plus therapeutic gardens. Physical plant amenities include an aqua therapy pool, robotics suite, rehabilitation gym, and fine dining.

Clinical staffing reaches 3 hours 21 minutes of nursing per resident per day, 11.1% above New York average, with registered nurse hours 31% above state average. CMS ratings are uniformly strong: 5-star overall (59.2% above state), health inspection 40.8% above average, staffing 11.1% above average, quality measures 26.6% above average.

Long-stay residents demonstrate exceptional clinical outcomes: functional decline of 7.5 (59% better than state), urinary tract infections at 0.3% (76% better), depression symptoms in 3.4% (81% better), and activities of daily living decline at only 9.5% (42% better than state). Short-stay rehabilitation discharge outcomes show 71.5% of residents achieving expected self-care ability (33% better than state).

Recent complaint investigations contradict the strong ratings. December 2025 inspection documentation identified four substantive deficiencies: environmental maintenance failures including ripped window screens and metal staples in resident units, failure to timely report three residents’ abuse allegations to the state within required timeframes, incomplete care plan revisions for residents’ behavioral and wandering risks, and food safety violations (bare hand contact with food, expired food items, staff without proper hair covering). An earlier January 2024 complaint survey identified six deficiencies: absent resident participation in care plan development, non-functional call bell in a resident’s bathroom, inaccurate baseline assessments, staffing information not posted, dietary preferences ignored, and missing hospice documentation. The inspection summary explicitly states the facility shows “a pattern of issues” and “deficiencies have persisted over time with similar themes, indicating ongoing challenges” in care planning, assessments, dietary compliance, and maintenance.

Zero critical violations and zero federal enforcement actions appear in regulatory history.

The facility generated $67.4 million in gross revenue in 2023 with a 4.1% profit margin, ranking 40th in New York for total revenue and demonstrating financial stability.

The community is situated in an urban area with a walk score of 91, indicating a walker’s paradise within easy reach of shops and services. Maimonides Medical Center lies 3.03 miles away.

The facility’s 5-star rating reflects strong clinical outcomes and staffing investment, yet recent inspections document persistent failures in care planning, abuse reporting, environmental maintenance, and food safety; operational deficiencies that suggest systemic supervision gaps despite high resource allocation.

Overview of Brooklyn Boulevard Alp

Brooklyn Boulevard Alp has been at 636 Louisiana Ave since 2005, so we’re talking roughly twenty years of assisted living in the same Brooklyn neighborhood. The building has 184 beds, and the surrounding area scores a 72 on the Walk Score scale, which is solidly walkable; most everyday errands don’t require a car, and that’s genuinely useful when families are in and out regularly.

What stands out about the clinical side is how much is actually on-site. There are doctors on premises, licensed nurses and CNAs around the clock, and licensed home care services built into the model. Rehabilitation and respite care are both available too, so the community isn’t just for people who need permanent placement; shorter-term stays are an option.

Three meals a day come out of the dining room, and the menus are put together with dietary restrictions, medical needs, and personal preferences in mind, not just a standard rotation. Activities run the range you’d expect: bingo, arts and crafts, fitness classes, current events, and the building itself has a library, beauty parlor, computer room, and transportation. There’s also a chapel and synagogue on-site for residents who want that.

State inspections here have tended to center on care planning and documentation.

Twenty years in, a medical team on the premises, and a full daily service structure under one roof; Brooklyn Boulevard Alp is a solid option for older adults in Brooklyn who don’t want to piece together care from multiple sources.

Frequently Asked Questions about Assisted Living in New York State

What's the difference between assisted living and memory care in New York State?

Assisted living in New York State supports residents with daily activities (bathing, dressing, medication management) while preserving independence. Memory care is a specialized form of assisted living for residents living with Alzheimer's or dementia, and adds 24/7 secured environments, dementia-trained staff, and structured routines designed to reduce confusion and wandering.

What's the difference between assisted living and a nursing home in New York State?

Assisted living in New York State is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.

Does New York State Medicaid cover assisted living?

New York State Medicaid does not directly pay room-and-board for assisted living, but most states (including New York State) offer Home and Community-Based Services (HCBS) waivers that can offset the cost of care services delivered inside a licensed community. Eligibility, waitlists, and covered services vary — check directly with the state Medicaid agency.

What is assisted living?

Assisted living combines private or semi-private apartments with help on activities of daily living (meals, bathing, dressing, medication management) and a social calendar — for residents who need some support but do not need 24/7 medical care.

How many assisted living facilities are listed on this page?

This page features 363 assisted living facilities in New York State. Use the filters and comparison tools above to compare ratings, amenities, and pricing.

How do I choose the right assisted living facility in New York State?

Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in New York State, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.

What should I look for when visiting assisted living facilities in New York State?

Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
