The inspection record traces a clear trajectory. August 2023 brought a serious finding: state surveyors investigated complaints of resident falls with injuries. The investigation substantiated that the facility had failed to maintain updated care plans with documented fall prevention measures for residents with fall history. One fall resulted in a resident death.

Within a year, the facility faced additional regulatory scrutiny: in September 2024, the State Fire Marshal documented six distinct fire safety deficiencies. These included combustible materials stored in boiler and mechanical rooms, electrical hazards from appliances connected to multiplug adapters, automatic fire doors failing to close properly, sprinkler system tamper switches left disconnected after backflow repair, missing annual fire extinguisher certification, and coded exit doors lacking required instructions. Most cited violations were corrected during the inspection itself; the sprinkler switches and door instructions remained outstanding.

The most recent full inspection on July 3, 2025, reported no deficiencies, which is a significant improvement.

Across three years, the facility recorded seven total deficiencies, averaging 2.3 per year compared to Washington’s average of 10.3.

Residents enjoy three daily meals plus snacks, housekeeping, medication management, bathing and dressing support, beauty and barber services, a library, activity lounge, and outdoor courtyards. Programming includes exercise classes, outings, bus tours, holiday celebrations, and games.
