Acts Retirement Services has run Westminster Village – Spanish Fort for 47 years now, out of a three-story building at 500 Spanish Fort Blvd in the Bridgehead neighborhood of Spanish Fort, Alabama. Joseph Meadows is the Executive Director. This isn’t a single-purpose nursing home. It’s part of a continuing care retirement community, which means nursing home care, assisted living, memory care, hospice, home care, and independent living all live under the same organization.
Don’t expect to walk to much because the Walk Score is 27. That is somewhat walkable at best, and most errands here mean getting in a car.
Average length of stay runs about 107 days, closer to four months than a permanent placement, and the community accepts both Medicare and private pay, the two payment paths that fit a short-term rehab stay best. Nursing staff provide close to 5 hours of total nursing care per resident each day. On-site, residents have access to licensed therapists, a nurse practitioner wellness center, and round-the-clock medical oversight, plus respite care and short-term rehab for people who need a defined recovery window rather than an open-ended stay.
Occupancy sits at 65% in a 60-bed building with studio, one-bedroom, and two-bedroom options. Residents also have an Active Resident Council, a standing group that meets to weigh in on policy and daily life rather than just live with whatever’s decided for them. State inspections here have tended to focus on routine areas like dietary and food service and resident rights and protections, the kind of themes that come up across nursing homes broadly.
Nearly five decades in the same location, paired with a rehabilitation-heavy care model and a resident group with an actual say in how things run, makes Westminster Village read less like a long-term holding facility and more like a structured stop on the way back to independence for a lot of residents.

Washington County Nursing Home is an 88-bed long-term care facility and county-operated governmental institution at 14600 St. Stephens Avenue in Chatom, Alabama, in Washington County. It’s administered by Teresa Grimes and functions as an integral part of Washington County Hospital, which has served the community since 1953. It accepts Medicare and Medicaid, and the current resident census (January 2026) shows 80 residents, with Medicaid covering 83.8 percent and private pay or other sources covering the rest.
The facility scored an overall 5-star rating by CMS, with every rating dimension above the Alabama average, the only facility in this review batch to achieve that across all four components. The overall score is 71.8 percent above the Alabama average, health inspection performance is 74.8 percent above average, staffing is 38.9 percent above average, and quality measures are 69.5 percent above average. The inspection record is extraordinary. Across three surveys over eight years: February 2018, February 2019, and June 2021, the facility accumulated just one total citation, placing it first out of 121 Alabama SNFs on both total citations and per-inspection citation rate. That single citation was a moderate Abuse/Neglect finding from the February 2018 survey, corrected promptly. No critical or serious citations have been issued, with no fines or penalties in the past three years. Quality measure performance for long-stay residents is similarly strong across most dimensions. Hospitalizations run at 0.94 per 1,000 resident days, 53 percent below the Alabama average of 2.00, indicating that chronic conditions are being managed proactively. ADL decline (9.4 percent vs Alabama average 13.9 percent) and walking ability decline (9.6 percent vs 16.1 percent) are both substantially better than state benchmarks, as are pressure ulcer (2.7 percent vs 5.9 percent) and UTI rates (2.3 percent vs 2.7 percent). Both pneumococcal and influenza vaccination rates reach 100 percent for long-stay residents. The composite high-risk clinical events score of 7.1 is 20 percent better than the Alabama average. Three areas warrant monitoring: weight loss at 9.6 percent (78 percent above Alabama average 5.4 percent), bowel/bladder incontinence at 21.7 percent (59 percent above average 13.6 percent), and falls with major injury at 4.4 percent (29 percent above average 3.4 percent). Given these figures, families of long-stay residents should ask specifically about fall prevention protocols and nutritional monitoring. Staffing is above average in total nursing hours, 4 hours 52 minutes per resident per day, 11 percent above the Alabama average, ranking 25th of 121 statewide. LPN hours stand out at 57 minutes per day, 21 percent above the state average. Weekend total nursing of 3 hours 34 minutes is 8 percent above average. One gap needs attention: weekend RN coverage at 8 minutes per day is 64 percent below the Alabama average of 22 minutes, the lowest weekend RN figure in this batch. Q2 2025 payroll data shows 103 total staff: 99 employees, 4 contractors (1.7 percent of hours) with 8.4 hours average shift. The 1.29:1 staff-to-resident ratio is below the Alabama average headcount ratio, though the nursing hours benchmarks remain above average, reflecting longer average shifts.
The campus’ specialty clinic access is a meaningful differentiator for a rural county-operated facility. On-site or affiliated services include cardiology, orthopaedics, pulmonary, radiology, vascular associates, a lab, a swing bed program, and the WCHNH Heritage Adult Day Haven, extending the facility’s services to day care for community members. Millry Family Care provides primary care access within the network.
Washington County Nursing Home is an exceptional option for Washington County and surrounding rural Alabama communities seeking a county-governed, 5-star rated facility. Its inspection record stands among the cleanest in the state, with quality results reflecting resident health outcomes instead of mere regulatory compliance. The RN weekend gap and the weight loss and incontinence metrics are the only material concerns in an otherwise highly favorable profile, though families of long-stay residents must ask about fall prevention protocols and nutritional monitoring.

Weekend total nursing at 3 hours 28 minutes is 5 percent above the Alabama average. RN hours at 24 minutes per day are 37 percent below the Alabama average, and weekend RN hours at 9 minutes are 59 percent below the state average of 22 minutes, the two staffing gaps families of clinically complex residents should raise on a tour. Q4 2025 payroll data shows 109 staff with 105 employees and only 4 contractors (0.7 percent contractor hours), one of the most stable employed workforces in this review.
The inspection record is the strongest component of the 5-star profile. Across five surveys spanning May 2018 through August 2023, Southern Springs Healthcare accumulated just 10 total deficiencies against an Alabama average of 17, at 1.3 per year against a state average of 2.1. No critical or serious citations have ever been issued. The August 2022 routine survey found zero deficiencies. The one substantiated complaint, investigated August 1 2023, found that a facility Admissions Coordinator had become the beneficiary of a resident’s $20,000 life and burial insurance policy and paid premiums without notifying the family. The employee was suspended, subsequently resigned, and corrective audits and staff training were implemented. No penalties have been issued in the past three years.
Quality measure highlights include emergency department visits for long-stay residents at 0.48 per 1,000 resident days (73 percent better than the Alabama average), falls with major injury 63 percent better than average, and UTI rates 81 percent better. Pressure ulcers at 12.1 percent (103 percent above the Alabama average) and weight loss at 10.5 percent (94 percent above average) are the two long-stay clinical outliers families should ask about. Frequently Asked Questions about Nursing Homes in Alabama
What's the difference between assisted living and a nursing home in Alabama?
Assisted living in Alabama is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.
Does Alabama Medicaid cover nursing home care?
Yes — Alabama Medicaid covers nursing home care for residents who meet income, asset, and medical-need eligibility requirements. Most CMS-certified nursing homes accept Medicaid as a primary payer once long-term-care eligibility is established.
What is nursing home care?
Nursing homes (also called skilled nursing facilities) provide 24/7 medical care from licensed nurses, rehabilitation services, and long-term custodial care for residents with significant health or functional needs.
How many nursing homes are listed on this page?
This page features 121 nursing homes in Alabama. Use the filters and comparison tools above to compare ratings, amenities, and pricing.
How do I choose the right nursing home in Alabama?
Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Alabama, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.
What should I look for when visiting nursing homes in Alabama?
Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
