Founded in 1909 and currently administered by Tunecia Campbell, the facility serves a mix of Medicare, Medicaid, and private-pay residents with an average stay of 101 days. Current occupancy is 90%, above the Ohio average of 78.7%.
The facility’s 2-star CMS rating reflects substantial compliance issues documented in recent complaint investigations.
In January 2025, inspectors found the facility failed to immediately report an abuse allegation from a ventilator-dependent resident against a night shift nurse. The resident reported being verbally abused, kicked, and pinched. A nurse practitioner learned of the allegation on December 31, 2024, but did not report it to administration that day. The facility finally notified state regulators four days later, on January 3, 2025, missing the required two-hour reporting deadline for physical abuse.
An August 2023 complaint inspection documented more serious care failures. Inspectors discovered maggots in the hairline of a resident with total ADL dependence who required ventilator support and had excessive oral secretions.
