Records and reports violations appear across three separate inspections from 2021 through 2025, suggesting either persistent documentation gaps or evolving regulatory expectations.

The environmental standards violations span facility maintenance, safety protocols, and other areas.

Resident services violations specifically address supervision gaps, a recurring theme across multiple inspection years. Personnel violations include background check process deficiencies. Over five years, the facility averaged 5 deficiencies annually, roughly 2 percent below the state average. However, this aggregate masks a striking year-to-year pattern: 2023 had zero deficiencies, 2024 had 16, and 2025 had one.

Two enforcement actions with stipulation agreements have been issued. The August 2022 stipulation (ACF-22-043) followed deficiencies in criminal history checks, provider notification responsibilities, resident services, and environmental standards. The March 2025 stipulation (ACF-25-022) followed the October 2024 relicensure survey deficiencies. Stipulations represent formal agreements with the state to correct serious or repeated violations.

Twelve lawsuits have been filed against the facility since 2010, all closed except one. The cases span negligence (Uhlitzsch, Chalk, Parker), tort claims (Franciosa, Cannella, Porter), and medical or professional malpractice (Ginty, Pruitt, and others).

Multiple cases involve estate administrators, suggesting resident death claims. The estate of John H. Pruitt, Jr. filed a medical/professional malpractice case in 2023, and this remains pending with a court appearance scheduled for July 29, 2026. The cumulative litigation and enforcement history over the past seven years indicates systemic compliance and care management challenges unresolved despite stipulation agreements.

Families should request documentation of corrective actions implemented since the March 2025 enforcement action and verify current compliance status.

Five inspections between May 2022 and June 2025 yielded a total of six deficiencies. Two inspections reported zero violations. The facility’s deficiency record by year shows: one in 2022, zero in 2023, two in 2024, and three in 2025. This represents an accelerating pattern of regulatory violations rather than improvement or stability.

Environmental standards deficiencies appear in all four inspections that identified violations.

May 2022 cited environmental standards (487.11 (h) (5)). June 2024 cited the same violation area twice, citing 487.11 (k) (1-3) and 487.11 (k) (15). June 2025 again cited environmental standards (487.11 (k) (1-3)).

The June 2025 inspection expanded the violation pattern beyond environmental issues to include food service (487.8 (e) (1)) and personnel (487.9 (a) (8)) deficiencies. This represents the first citation of violations outside environmental standards.

A March 2023 complaint investigation found no substantiated issues.

The facility averaged 1.5 deficiencies per year, placing it 71 percent better than New York’s state average of 5.1. One complaint was filed, representing 3.2 per 100 occupied beds versus a state average of 49.9 per 100. No enforcement actions or fines appear on record.

One negligence lawsuit was filed in 2015. Stephen Levine, acting as administrator of an estate, sued Camphill Ghent, Inc. and Funplex, Inc. The case is closed. The administrator designation suggests a resident death claim.

Because of the worsening deficiency trend in June 2025 families should ask about corrective actions and their outcomes.
