One hundred percent occupancy and a 21-day average stay. Those two numbers define what Advanced Health Care of Albuquerque is: a short-stay post-acute rehabilitation facility running at capacity. Every one of its 47 beds at 2701 Richmond Dr NE in Albuquerque’s Duke City Industrial Area is filled, and the residents cycling through them are there for a defined, clinical purpose before returning home.
The rehabilitation infrastructure supports that model. Inpatient rehabilitation services are on-site. Twenty-four-hour nursing care is in place. Total nursing staff time runs 3 hours and 56 minutes per resident per day, with registered nurses accounting for 1 hour and 8 minutes of that directly. Private suites house residents during their stay; a library, beauty salon, private dining room, and fireplace fill out the setting. Restaurant-style dining draws on fresh and seasonal ingredients.
State inspections have tended to focus on medication management, infection prevention and control, care planning and documentation, and dietary and food service. Medicare and private pay are accepted; Medicaid is not, though at 21 days average and a near-total Medicare payer mix, this is rarely a placement issue for the rehabilitation population this facility serves. Administrator Edwardo Rivera leads operations under the Advanced Health Care brand.
What Advanced Health Care of Albuquerque is, plainly, is a tight, full-capacity short-stay nursing home with a focused clinical identity: post-acute rehabilitation, private suites, and a premium amenity footprint, in a moderately walkable Albuquerque neighborhood 3 miles from the city center.
Farmington’s San Juan Care Center operates as a 93-bed skilled nursing facility in a pedestrian-friendly neighborhood where most essential services are reachable on foot. The facility admits residents across the care spectrum, from those managing short-term recovery to those requiring ongoing medical supervision. The average resident tenure of 81 days suggests a steady mix of both populations, neither heavily weighted toward either trajectory. Current occupancy sits at 44%, meaning the facility has room available and can accommodate new admissions without operational strain.
Nursing staff composition tells a practical story about daily operations. The facility deploys registered nurses for roughly 37 minutes per resident per day, with nurse aides, who typically handle bathing, dressing, and mobility assistance, providing more than two hours of direct contact time. Total nursing hours (RN, LPN, and aide combined) land at 3 hours and 25 minutes per resident daily.
For a short-term rehabilitation patient, this level of coverage supports wound care monitoring, medication management, and therapy coordination. For a longer-stay resident, it means consistent monitoring without intensive ICU-level staffing.
San Juan’s clinical offerings are straightforward. The facility provides on-site rehabilitation services and wound care, has a physician available to the staff, and offers respite stays for families managing care at home. Dietitians manage meal planning around both medical restrictions and religious or cultural dietary practices, which matters tangibly when someone’s beliefs shape what they’ll eat. The facility also maintains basic services like barber and beauty, amenities that affect quality of life in ways standard clinical metrics miss.
Payers can use Medicare or Medicaid here, or arrange private payment. The facility accepts all three pathways. Administratively, the facility operates under New Mexico’s regulatory requirements and maintains its licensure as a skilled nursing provider.
The four years of regulatory history on file reflect standard state oversight mechanisms focused on recordkeeping, sanitation standards, and care documentation. This is the operational baseline for any facility in the state.
Clayton Nursing and Rehab Center sits at 419 Harding Street in downtown Clayton, a rural seat of Union County in northeastern New Mexico. Operated by 419 Harding Street Operations LLC under administrator Carolyn Kear, it is a 45-bed skilled nursing facility built around short-term post-acute care. Amenities include an in-house pharmacy, professional podiatry and vision services, a beauty and barber shop, an open courtyard, and a named Family Cares Program designed to keep family members engaged in care decisions. The facility accepts Medicare, Medicaid, and private pay, and its doors are open to adults 18 and older who need skilled nursing or specialized rehabilitation.
The clinical lineup here is what you’d expect from a facility whose admitted population is predominantly Medicare-covered: orthopedic rehab, stroke and neuro recovery, cardiac care, sub-acute management, wound care, IV therapy, pulmonary care, and bariatric care are all listed services. Physical therapy, occupational therapy, and speech-language pathology are represented in the staffing mix. Total nursing care runs 3 hours and 48 minutes per resident per day across a 24-hour staffing model, with 50 total staff including 8 registered nurses, 5 LPNs, and 15 certified nursing assistants. A nurse practitioner and a mental health service worker are also on the payroll, a pairing that’s not universal in facilities this size.
Thirty-one of 45 beds are currently filled, an occupancy rate of 69%, which suggests placement is generally available without a waitlist. The average stay runs about 280 days, a figure that reflects a mix: some residents cycle through quickly on Medicare rehabilitation stays, while others settle into longer Medicaid-covered tenure.
The Walk Score of 63 places the surrounding area in moderately walkable territory. Clayton’s city center and the facility’s address are essentially the same point, which makes access straightforward for families coming in from town.
State inspections over the four-year data window have been conducted by the New Mexico Department of Health’s Division of Health Improvement. The inspection record shows no critical or serious citations during that period; findings have been confined to moderate categories covering areas such as infection control, nutrition, and quality of life standards.
For adults leaving a hospital after surgery, cardiac events, or neurological episodes, Clayton Nursing and Rehab Center offers a focused short-stay rehabilitation model in a small-town facility with direct access to a diversified clinical team.
With a solid presence serving the community for 34 years, Cedar Ridge Inn is a skilled nursing home on Saguaro Trail in Farmington, New Mexico. Operated by Farmington Operations Holdings LLC, the 101-bed facility has a 90 percent occupancy rate. It accepts Medicare, Medicaid, and private pay, so families have several ways to afford care and support.
The home specializes in rehabilitation services, making it a pragmatic choice for residents convalescing from surgery, acute illness, or hospitalization. Averaging a stay duration of 112 days, Cedar Ridge Inn serves short-term rehabilitation and longer-term nursing care residents. Daily nursing support is significant, with total care averaging 3 hours 46 minutes per resident day, providing valuable hands-on attention and clinical oversight all day. The Farmington location carries practical advantages for occupants and families. The area scores 71 on the Walk Score, indicating that most errands and essentials are walkable, convenient for visiting family, and for residents who can navigate the neighborhood. Cedar Ridge Inn operates as a skilled nursing community focused on delivering rehabilitation and nursing care. Its longevity in the area implies stability and established relationships in the local healthcare landscape.
Grants Wellness & Rehabilitation operates as a mid-sized skilled nursing facility in Grants, a rural area of northwestern New Mexico where most errands require a car. The facility sits at 840 Lobo Canyon Road in a somewhat walkable neighborhood (Walk Score 25), though the isolation that defines rural New Mexico is evident in how residents and families will need transportation for most daily activities.
The average resident stays 83 days, which points to Grants Wellness as primarily a short-term rehabilitation hub rather than a long-term residential community. That’s a meaningful distinction for families evaluating whether this is the right fit. If your loved one needs post-acute recovery after surgery or hospitalization, the short-stay model makes sense. If you’re looking for a permanent placement, this isn’t the setting.
The facility’s staffing allocation bears scrutiny. Residents get roughly 5 hours and 14 minutes of total nursing care daily, with registered nurses on staff for 34 minutes per resident per day, nurse aides for 2 hours and 42 minutes, and LPNs for another 34 minutes. That’s a significant LPN presence alongside RN and aide support. The inclusion of a doctor on staff and round-the-clock staffing is standard for nursing homes but worth noting. Rehabilitation services and respite care are available in-house, which means someone recovering from a fall or orthopedic surgery isn’t shuffled off-site for therapy.
The facility manages dietary care around both medical requirements and faith-based preferences. Meals are built to be nutritionally sound. No pets are permitted, which eliminates that variable for families considering placement. The facility accepts Medicare, Medicaid, and private pay; a straightforward approach to coverage that keeps the financial picture clear.
The data suggests a facility doing its work without fanfare. Moderate occupancy, stable but unspectacular occupancy, and the demographic realities of rural New Mexico health care infrastructure all converge on a picture of a place that serves its function competently but operates in a market where demand isn’t outpacing supply.
Gallup sits at the intersection of multiple transportation corridors in northwestern New Mexico, and that geography shows up in the data. Gallup Nursing & Rehabilitation, located at 306 East Nizhoni Boulevard, operates in a very walkable area (Walk Score 79), which is genuinely unusual for a nursing home and functionally significant.
Visiting family members can navigate downtown Gallup on foot. Residents stable enough to walk can access nearby services without relying entirely on facility transport. That’s a material advantage over rural facilities where everything requires a car.
The 62-bed facility runs at 92 percent occupancy. Full beds matter in the skilled nursing market because they signal consistent demand, and they also signal the facility isn’t maintaining intentionally low census to manage workload. With an average stay of 61 days, Gallup Nursing operates primarily as a post-acute rehabilitation hub. Your loved one stays long enough to stabilize and regain function after surgery or hospitalization, then transitions back home or to another setting.
It’s a throughput model, and the numbers suggest it’s working. The facility fills beds steadily enough that it maintains near-maximum occupancy while rotating through short-term patients.
Residents receive 3 hours and 30 minutes of total nursing care daily. Breaking that down: registered nurses spend 49 minutes per resident per day, nurse aides contribute 2 hours and 9 minutes, and LPNs add 12 minutes.
That staffing architecture leans heavily on aide support, which is standard in rehabilitation settings where physical assistance and monitoring matter more than complex medical oversight. The presence of a doctor on staff and 24-hour nursing coverage meets the baseline for skilled care. Short-term rehabilitation and respite care are available in-house.
The facility attends to dietary needs by providing nutritionally balanced meals that accommodate medical restrictions and faith-based preferences. No pets. The payment structure accepts Medicare, Medicaid, and private pay, which streamlines the financial conversation.
The high occupancy coupled with the short-stay model and strong walkability metric creates a picture of a facility that has cracked the problem of operational stability in a smaller market. It fills beds, moves patients through recovery, and locates itself in a place where families can actually reach it without depending entirely on a car.

It sits in a really walkable neighborhood where errands and essential services are within walking distance, a feature that can simplify visits for family members. Medicare and private pay are accepted, giving families flexibility in funding care costs.
The short average stay duration of 19 days indicates this is primarily a rehabilitation and post-acute care setting. It’s tailored to support occupants healing from surgery, hospitalization, or illness before returning home or to longer-term care. Rehabilitation services are a core focus, and the facility offers respite care for families needing short-term support. The home operates with 24-hour staffing and maintains a total of roughly 5 hours 38 minutes of nursing care per resident day, with registered nurses, nurse aides, and licensed practical nurses all contributing to daily care. A certified dietitian manages the kitchen and oversees nutritionally balanced meals tailored to meet individual health needs. With an occupancy rate around 44 percent, the community has room for new residents.
State inspections over recent years have flagged issues primarily focused on medication management protocols and storage practices, along with food safety, respiratory care supply handling, and incident documentation completeness.
To know how Farmington Wellness & Rehabilitation ensures accuracy and safety in medication administration and record-keeping, families may want to ask about such areas when touring.

Frequently Asked Questions about Nursing Homes in New Mexico
What's the difference between assisted living and a nursing home in New Mexico?
Assisted living in New Mexico is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.
Does New Mexico Medicaid cover nursing home care?
Yes — New Mexico Medicaid covers nursing home care for residents who meet income, asset, and medical-need eligibility requirements. Most CMS-certified nursing homes accept Medicaid as a primary payer once long-term-care eligibility is established.
What is nursing home care?
Nursing homes (also called skilled nursing facilities) provide 24/7 medical care from licensed nurses, rehabilitation services, and long-term custodial care for residents with significant health or functional needs.
How many nursing homes are listed on this page?
This page features 56 nursing homes in New Mexico. Use the filters and comparison tools above to compare ratings, amenities, and pricing.
How do I choose the right nursing home in New Mexico?
Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in New Mexico, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.
What should I look for when visiting nursing homes in New Mexico?
Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
