The most recent inspection in June 2025, a routine survey and a complaint investigation, identified 15 deficiencies, including failure to protect residents from abuse by both a resident and a staff member, with substantiated abuse by a registered nurse who was terminated. Over the years, the community has also accumulated a total of 51 deficiencies, which is significantly higher than the state average of 14. Earlier inspections also showed multiple recurring concerns, including incomplete care plans, inadequate medication management, insufficient abuse investigations, and problems with resident supervision and hygiene. However, no license suspensions or fines were reported

Mission at Community Living Rehabilitation Center is a 46-bed skilled nursing and memory care facility in Centerfield, Utah. Licensed since 1994 and serving residents in Sanpete County, the facility accepts Medicare and Medicaid. It offers skilled nursing, short-term rehabilitation, and memory care, with private suites and individualized therapy programs.
The inspection record of four years shows meaningful regulatory challenges. 27 deficiencies were recorded across three inspections, nearly double the Utah average of 14, 94 percent above the state’s average of 3.5 deficiencies annually. The most consequential findings stem from a May 2023 resident rafting outing when multiple staff members, including a maintenance and an activities staffer, consumed alcohol while transporting residents, with one staff member subsequently arrested for DUI in a facility vehicle. Two substantiated complaint investigations at the June 2025 survey cited the facility’s failure to timely report the incident, conducting an incomplete internal investigation with no ongoing substance abuse training to staff afterward. The September 2023 complaint surveys added deficiencies for untimely abuse reporting across five separate resident incidents, inadequate physician and pharmacist oversight, missing discharge summaries, food safety violations, and having no Legionella testing program. The February 2022 routine inspection cited incomplete care plans and inaccurate medication records. The June 2025 routine visit found the facility largely compliant, citing only one documentation gap. No fines or enforcement actions were reported. Staffing hours run below Utah benchmarks across all measured categories. Registered nurse hours reach 44 minutes per resident per day, 38 percent below the state average of 1 hour and 11 minutes. Certified nursing assistant hours are 1 hour and 44 minutes per day, 27 percent below average. Weekend nursing coverage is 2 hours and 31 minutes per day, 28 percent below the state figure.
