East Portland Care Center is a 30-bed skilled nursing, rehabilitation, long-term care, and memory care facility at 34 NE 20th Avenue, Portland, Multnomah County, Oregon. Operated by Sapphire At Rose City, LLC, the community provides skilled nursing, rehabilitation, memory care, specialty care, home care, assisted living, hospice, and transitional independent living with 24/7 nursing, therapy services, daily living assistance, nutritional support, and activities programming.
The regulatory record documents systemic and persistent care quality and compliance gaps. Over four years, East Portland Care Center logged 47 deficiencies; 11.8 annually, 76 percent above Oregon’s 6.7-deficiency benchmark.
The September 16, 2025, complaint investigation issued two initial comment citations with zero corrective action documented. The February 26, 2025, complaint investigation revealed fourteen deficiencies in notice requirements, activities, activity professional qualifications, safety hazards, dialysis, RN coverage, staffing information, medication storage, food sanitation, employee background checks, nursing services, and administrative rules; many were corrected by March 22, 2025, though some persisted through April 3, 2025. A March 8, 2024, complaint investigation found five deficiencies in bed hold policy notification and quality of care spanning bowel care, wound care, nutrition, comprehensive assessment, medication regimen review, RN coverage, medication errors, records accuracy, and training; some were corrected after initial visit but not at follow-up. An October 20, 2023, complaint investigation documented twenty deficiencies covering notification of physician-relevant changes, safe environment, comprehensive and accurate assessment, wound care, nutrition monitoring, RN coverage, drug regimen review, medication errors at or above the 5 percent threshold, resident records, and nurse aide training; many were corrected by November 12, 2023, but remained uncorrected by December 5, 2023.
Six focused infection control surveys from October through December 2022 repeatedly cited failure to report complete COVID-19 information to the CDC’s National Health Safety Network, with citations uncorrected throughout. Medication errors meeting or exceeding the 5 percent error rate flagged in the October 2023 inspection point to systemic gaps in pharmaceutical safety oversight.
Complaint investigations were unsubstantiated. No fines or license actions are recorded.
The community provides skilled nursing, rehabilitation, memory care, and specialty services with 24/7 nursing, therapy, daily living assistance, nutritional support, and activities. Private pay is required; Medicaid and Medicare are not accepted.
Families should ask about corrective actions taken following the October 2023 inspection, regarding medication errors, RN coverage, comprehensive assessments, and wound care, and request verification on tour that quality-of-care measures are in place and functioning.
Cascade Terrace Post Acute is a 105-bed post-acute skilled nursing and memory care facility at 5601 SE 122nd Avenue, Portland, Multnomah County, Oregon, 8.3 miles from downtown. Operated by Cascade Terrace SNF Healthcare, LLC, the community provides 24-hour skilled nursing with rehabilitation, memory care, therapy and assessment services six to seven days weekly, case management, discharge planning, and specialized programs including joint replacement and orthopedic rehabilitation, stroke rehabilitation, cardiac rehabilitation with CHF protocol, fall prevention, pulmonary care, post-operative care, pain management, infusion therapy, vascular wound management, renal disease services, tracheostomy care, cancer recovery, bariatric care, LVAD support, and palliative and hospice care.
The regulatory record documents severe and persistent care quality and operational failures spanning medication management, staffing, abuse prevention, and compliance systems. Over two years, Cascade Terrace logged 61 deficiencies; 30.5 annually, 355 percent above Oregon’s 6.7-deficiency benchmark.
A January 31, 2024, complaint investigation and state licensure survey revealed twenty-one deficiencies spanning vaccine consent, reasonable accommodations, change notification, abuse prevention and injury investigation, care plan implementation and revision, elopement prevention, accident hazards, staffing sufficiency, nurse aide training, behavioral health services, medication error prevention, binding arbitration, infection control, and in-service training. Several immediate jeopardy situations were identified and abated during this inspection. An April 28, 2025, complaint investigation documented twenty-one deficiencies in medication self-administration assessment, privacy and confidentiality, comprehensive and timely assessments, accurate MDS coding, ADL care, activity programming, quality of care, accident hazards, dialysis, RN staffing, medication storage, dietary needs, infection control, and payroll-based journal reporting; many were corrected by follow-up but some persisted uncorrected.
A January 23, 2025, survey cited five deficiencies in ADL care, quality of care, and fall prevention care plan implementation with partial correction. A November 6, 2025, survey identified four deficiencies including failure to clarify chemotherapy medication orders resulting in adverse outcomes, with multiple citations uncorrected at visit. Complaint surveys on September 23, 2025, March 6, 2025, August 21, 2024, and March 12, 2024, noted citations uncorrected at visit. Two January 2024 focused surveys cited COVID-19 NHSN reporting failures.
The January 2024 immediate jeopardy findings, the April 2025 twenty-one deficiency cluster, and the November 2025 chemotherapy medication order failure resulting in patient harm, constitute a pattern of fundamental care and systems failure.
While some facility responsiveness to revisit audits was documented, the persistence of uncorrected deficiencies across complaint investigations from January 2024 through November 2025 indicates incomplete corrective implementation and sustained operational fragmentation. No fines or license actions are recorded.
The facility provides post-acute and skilled nursing care with rehabilitation, memory care, and specialized services. Private pay is required; Medicaid and Medicare are not accepted. Porthaven Post Acute is a 99-bed post-acute skilled nursing and memory care facility at 5330 NE Prescott Street, Portland, Multnomah County, Oregon, 5.6 miles from downtown. Operated by Porthaven SNF Healthcare, LLC, the community provides 24-hour skilled nursing with rehabilitation, memory care, therapy and assessment services six to seven days weekly, case management, discharge planning, and specialized programs including joint replacement and orthopedic rehabilitation, stroke rehabilitation, cardiac rehabilitation with CHF protocol, fall prevention, pulmonary care, post-operative care, pain management, infusion therapy, vascular wound management, renal disease services, tracheostomy care, cancer recovery, bariatric care, LVAD support, and palliative and hospice care. The facility holds a 4-Star Quality Measures Medicare Rating.
The regulatory record documents critical and escalating failures in resident safety, care quality, and fundamental operational systems. Over two years, Porthaven logged 36 deficiencies, averaging 18 annually, 169 percent above Oregon’s 6.7-deficiency benchmark.
An April 8, 2024, focused infection control survey identified one COVID-19 NHSN reporting deficiency with potential for more than minimal harm. A May 13, 2024, complaint investigation documented four deficiencies in failure to maintain adequate staffing to meet resident care needs timely. Multiple residents reported delays and lack of assistance. Staff reported chronic understaffing and high workload.
An August 30, 2024, complaint investigation revealed four deficiencies, the most serious being failure to protect a resident from sexual abuse by another resident, accompanied by failures in care planning, grievance follow-up, discharge notification, bed hold policy, assessment accuracy, ADL care, quality of care, fall prevention, respiratory care, therapy services, hospice coordination, infection control, hand hygiene, and nurse call systems. An April 9, 2025, complaint investigation cited five deficiencies spanning wound assessment documentation, infection control, Ombudsman notification, bed hold policy, resident assessment accuracy, antibiotic treatment timeliness, nail care provision, fall prevention, nurse call system function, physical therapist employment, hospice coordination, and hand hygiene and PPE protocols. An April 16, 2025, complaint investigation documented four deficiencies including failure to protect a resident from physical abuse by another resident resulting in injury and prolonged pain, infection control failures, failure to employ physical therapist, and care planning and abuse prevention deficiencies. An August 7, 2025, complaint investigation identified four deficiencies: inappropriate discharge of a resident to a home with no running water, rats, and no heat; failure to include primary physician in discharge planning.
The resident subsequently required placement in another skilled facility. An October 13, 2025, complaint investigation found no deficiencies. A December 8, 2025, complaint investigation revealed ten deficiencies in resident and family grievance response, environmental safety and comfort, activities programming, quality of care, mobility maintenance, accident hazard supervision, food sanitation, and administrative rules.
