Cascade Terrace Post Acute sits in outer Southeast Portland at 5601 SE 122nd Avenue, a 105-bed skilled nursing and palliative care community operated by Cascade Terrace Snf Healthcare, LLC under administrator Blake Epp. With a Walk Score of 10, transportation to and from this location will require a car or arranged service for virtually all trips. Occupancy is 56%, and the average resident stay runs 78 days — numbers that point to a facility serving a mix of short-stay rehabilitation admissions and longer-term residents.
Therapy runs six to seven days a week., and beyond 24-hour skilled nursing, short-term rehab, hospice, and palliative care, the facility carries programs covering joint replacement and orthopedic rehabilitation, stroke and cardiac rehabilitation with CHF protocol, fall prevention, pulmonary care, post-operative care, pain management, infusion therapy, vascular wound management, renal and digestive disease services, cancer recovery, and bariatric care. Physical, Occupational, and Speech Therapy Services are on-site, as are pharmacy, laboratory, and X-ray services. The inclusion of Left Ventricular Assist Device (LVAD) support indicates clinical capacity for residents managing complex cardiac conditions.
Nursing staffing per resident per day: 33 minutes from registered nurses, 50 minutes from licensed practical nurses, and 3 hours and 16 minutes from certified nurse aides. Private and semi-private rooms are available alongside transportation assistance and a monthly activities calendar. Medicare, Medicaid, and private pay are accepted.
State inspections have touched on medication management, infection control, staffing credentials, care planning and documentation, and resident rights. The Oregon Department of Human Services, Aging and People with Disabilities is the regulatory authority.
Cascade Terrace Post Acute is a high-acuity post-acute facility in Portland’s outer southeast with a notably broad clinical program list, LVAD support capability, and 24-hour nursing coverage — a combination that positions it for residents with complex, medically intensive care needs requiring more than standard rehabilitation services.
Gracelen Terrace has been providing top-notch service since 1962 as a private senior facility in Portland. They prioritize privacy and take pride in their beautifully landscaped exteriors, providing an attractive and comfortable setting for their residents. They offer intermediate care, catering to each individual resident’s unique needs. They offer various specialized services such as podiatry, orthotics and prosthetics, dental, ophthalmology, dermatology, psychiatry, audiology, and more.
Its staff is committed to serving people of all backgrounds and languages. To ensure effective communication and care for all, they have interpreter services contracts in place. Residents also enjoy an engaging lifestyle with exciting excursions, entertaining events, and programs that stimulate their emotional and physical well-being. There are intellectual activities available to challenge the mind and help residents maintain their cognitive abilities. The staff values every resident and is dedicated to providing the ideal retirement lifestyle.
East Portland Care Center occupies a single-story building at 34 NE 20th Ave in Portland’s Kerns neighborhood, about 2 miles from the city center. The 30-bed nursing home is operated by Sapphire at Rose City, LLC, under administrator Marcus Roshak. With a Walk Score of 93, the facility sits in what qualifies as a walker’s paradise: most daily errands are reachable on foot. Providence Portland Medical Center is roughly 1.6 miles out.
The care focus here is post-acute. Services include short-term rehabilitation, respite, palliative care, and hospice, all under 24-hour nursing coverage. The facility’s Transitional Independent Living program positions it specifically for residents working toward a return to everyday life after a health episode. Nursing staffing runs at 1 hour per day per resident for RNs, 1 hour and 10 minutes for LPNs, and 3 hours and 48 minutes for certified nurse aides, with aides making up the bulk of hands-on daily contact.
Meals are dietitian-guided and served in a shared dining room, calibrated to both clinical and personal preferences. East Portland Care Center accepts Medicaid, which matters for families mapping out a longer funding picture.
At 30 beds, this is a small facility with a narrow, deliberate focus. The combination of short-stay rehabilitation programming, around-the-clock nursing, and a highly walkable urban location makes it a practical fit for adults who need skilled post-acute care and are planning to return home.
Licensed with 47 beds, Willamette View Terrace is a residential care community in Portland, Oregon, that has been operating for 26 years. It offers independent living and personal care. With a Walk Score of 72, its surrounding area is highly walkable, making it easier to access amenities and errands on foot.
Skilled nursing care is also provided for those who need ongoing medical support, post-acute recovery, and assistance with daily living. Dining, activities, and access to amenities that support both recovery and quality of life are also offered. Moreover, multiple dining venues, including Riverview, Elk Rock, White Oak Grill, Marketplace, and Waterfalls Cafe, are available, offering varied meal options. The community also features a fitness center, library, and outdoor spaces, which combine clinical care with daily comfort.
The state’s Department of Human Services, Aging and People with Disabilities has identified community efforts aimed at maintaining compliance with care and safety standards. Moreover, earlier inspections documented substantial improvement. The most recent kitchen inspection also reported steps taken to address those concerns.
If you are looking at Reedwood Post Acute, you are looking at a 64-bed setup situated at 3540 SE Francis Street in southeast Portland. With a Walk Score of 58, it has decent accessibility for the area. It is run by Creston Snf Healthcare, LLC, and managed by administrator Andrew Thomas. With a 72% occupancy rate and an average resident stay of 105 days, this is clearly a facility designed for people who need more than just a quick pit stop; it is built for those who require a longer, more structured recovery period.
When it comes to the actual care, they provide 24-hour skilled nursing and palliative options, with rehabilitation services available six to seven days a week. Their clinical reach is broad, covering everything from orthopedic and joint replacement recovery to cardiac and stroke rehabilitation, pulmonary care, and pain management. They also manage specialized needs like vascular wounds, renal and digestive health, cancer recovery, and bariatric support.
What is particularly noteworthy is their ability to support patients with Left Ventricular Assist Devices (LVAD). This capability allows them to handle complex cardiac cases that many other facilities simply aren’t equipped to take on. Because physical, occupational, and speech therapy, plus pharmacy, lab, and X-ray services, are all available on-site, they are able to keep clinical care centralized.
On a daily basis, residents receive about 41 minutes of care from registered nurses, 36 minutes from licensed practical nurses, and 2 hours and 52 minutes of support from certified nurse aides. For living arrangements, they offer both private and semi-private rooms, and they provide transportation assistance and a calendar of activities to keep residents engaged. Financially, they accept the standard options: Medicare, Medicaid, and private payment.
Regulatory oversight is handled by the Oregon Department of Human Services, Aging and People with Disabilities. Recent inspections have focused on the standard operational areas, including infection control, staffing credentials, care planning, food service, and resident rights.
Reedwood functions as a vital hub in southeast Portland for individuals with complex post-acute needs. Between their clinical capability to support LVAD patients and the capacity for longer, extended stays, the facility stands out as a practical option for anyone needing specialized attention beyond what a typical short-term rehabilitation stay might offer.
Located in Portland’s Powellhurst-Gilbert neighborhood on SE Bush Street, Avalon Care Center Portland is a 44-bed nursing home operated by Avalon Care Center – Portland, LLC under administrator Danielle Meza. The building opened four years ago and covers three care types in one setting: nursing home care, memory care, and hospice. Adventist Health Portland sits 1.76 miles away, which is a practical proximity for a nursing facility serving residents with complex medical needs. The neighborhood scores 61 on the Walk Score scale, making it moderately walkable for visiting family and staff.
The clinical services go beyond basic skilled nursing; rehabilitation services, short-term rehab, and respite care are all available. A Bariatric Care Program is a named offering for residents with specialized weight-management care needs. Licensed nurses and CNAs provide 24-hour coverage.
On the staffing side, nurse aides contribute 4 hours 20 minutes of direct care per resident per day, with RNs adding 30 minutes and LPN/LVNs 46 minutes. Weekend total nursing comes to 5 hours 2 minutes per resident per day, holding coverage steady across the full week.
The activities program has real breadth: a garden group, exercise classes, sensory activities, cooking classes, musical entertainment, and special events, all run on the schedule. A beauty salon and daily activities program are named structured offerings, and resident and family support services are in place. Dining menus are built around individual resident preferences and seasonal produce. A courtyard and patio provide outdoor space.
Avalon Care Center Portland accepts Medicaid, and with hospice and memory care layered onto its skilled nursing foundation, it serves residents across a wider range of clinical stages than a typical short-stay rehabilitation facility.
Porthaven Post Acute occupies a 99-bed building on NE Prescott Street in Portland, Oregon, about 5.6 miles from the city’s downtown core. Porthaven SNF Healthcare, LLC operates the facility. The surrounding neighborhood is car-dependent (Walk Score 4), so visiting family and outgoing residents should plan on driving for most errands.
The clinical program list here is long and deliberately structured around complex post-acute cases. Physical, occupational, and speech therapy run six to seven days a week.
Beyond the standard rehabilitation menu, the facility handles cardiac rehabilitation under a CHF protocol, LVAD support, intravenous/infusion therapy, vascular wound management, pulmonary care, renal disease services, digestive disease services, cancer recovery, and bariatric care, alongside coordinated palliative and hospice care. Pharmacy, lab, and X-ray services are on-site. An average stay of 67 days fits the short-to-medium-term rehabilitation arc that drives most admissions here.
Current occupancy sits at 54%, with 53 of 99 beds filled. Both private and semi-private rooms are available. Transportation assistance and a monthly activities calendar round out the resident experience.
Staffing totals 5 hours and 31 minutes of nursing care per resident per day. Registered nurses account for 25 minutes of that; LPN/LVN staff cover an additional 53 minutes. The facility runs 24-hour staffing and delivers therapy on most days of the week.
Oregon’s Department of Human Services, Aging and People with Disabilities, conducts inspections at the facility. Inspection themes have touched on infection prevention and control, care planning and documentation, and resident rights and protections.
Medicare, Medicaid, and private pay are all accepted. For adults navigating recovery from cardiac events, strokes, orthopedic procedures, or other medically complex episodes, Porthaven’s specialty-specific program depth and multi-payer access make it a functionally distinct option within the Portland post-acute market.

Operated by Sapphire At Rose City, LLC, the community provides skilled nursing, rehabilitation, memory care, specialty care, home care, assisted living, hospice, and transitional independent living with 24/7 nursing, therapy services, daily living assistance, nutritional support, and activities programming.
The regulatory record documents systemic and persistent care quality and compliance gaps. Over four years, East Portland Care Center logged 47 deficiencies; 11.8 annually, 76 percent above Oregon’s 6.7-deficiency benchmark.
The September 16, 2025, complaint investigation issued two initial comment citations with zero corrective action documented. The February 26, 2025, complaint investigation revealed fourteen deficiencies in notice requirements, activities, activity professional qualifications, safety hazards, dialysis, RN coverage, staffing information, medication storage, food sanitation, employee background checks, nursing services, and administrative rules; many were corrected by March 22, 2025, though some persisted through April 3, 2025. A March 8, 2024, complaint investigation found five deficiencies in bed hold policy notification and quality of care spanning bowel care, wound care, nutrition, comprehensive assessment, medication regimen review, RN coverage, medication errors, records accuracy, and training; some were corrected after initial visit but not at follow-up. An October 20, 2023, complaint investigation documented twenty deficiencies covering notification of physician-relevant changes, safe environment, comprehensive and accurate assessment, wound care, nutrition monitoring, RN coverage, drug regimen review, medication errors at or above the 5 percent threshold, resident records, and nurse aide training; many were corrected by November 12, 2023, but remained uncorrected by December 5, 2023.
Six focused infection control surveys from October through December 2022 repeatedly cited failure to report complete COVID-19 information to the CDC’s National Health Safety Network, with citations uncorrected throughout. Medication errors meeting or exceeding the 5 percent error rate flagged in the October 2023 inspection point to systemic gaps in pharmaceutical safety oversight.
Complaint investigations were unsubstantiated. No fines or license actions are recorded.
The community provides skilled nursing, rehabilitation, memory care, and specialty services with 24/7 nursing, therapy, daily living assistance, nutritional support, and activities. Private pay is required; Medicaid and Medicare are not accepted.
Families should ask about corrective actions taken following the October 2023 inspection, regarding medication errors, RN coverage, comprehensive assessments, and wound care, and request verification on tour that quality-of-care measures are in place and functioning.

Frequently Asked Questions about Nursing Homes in Portland, OR
What's the difference between assisted living and a nursing home in Oregon?
Assisted living in Oregon is a residential model focused on housing, hospitality, and help with daily activities. Nursing homes (skilled nursing facilities) provide 24/7 medical care from licensed nurses for residents with significant health needs, and are regulated more strictly under both state and federal CMS rules.
Does Oregon Medicaid cover nursing home care?
Yes — Oregon Medicaid covers nursing home care for residents who meet income, asset, and medical-need eligibility requirements. Most CMS-certified nursing homes accept Medicaid as a primary payer once long-term-care eligibility is established.
What is nursing home care?
Nursing homes (also called skilled nursing facilities) provide 24/7 medical care from licensed nurses, rehabilitation services, and long-term custodial care for residents with significant health or functional needs.
How many nursing homes are listed on this page?
This page features 21 nursing homes in Portland, OR. Use the filters and comparison tools above to compare ratings, amenities, and pricing.
How do I choose the right nursing home in Portland, OR?
Start by matching the level of care offered to the resident's current and anticipated needs, then compare licensing status, staff-to-resident ratios, recent inspection results, and pricing. Tour at least two or three communities in Portland, OR, talk to current residents and families, and confirm what is included in the base rate versus billed as add-on services.
What should I look for when visiting nursing homes in Portland, OR?
Pay attention to staff interactions with residents, cleanliness and odor, food quality at meal times, the activity calendar, and how questions about pricing and care plans are answered. Ask to see the most recent state inspection report, the move-out / level-of-care-change policy, and a sample monthly bill that lists every fee.
