Licensed nurses and CNAs provide 24-hour staffing. Private-pay only; starting rate $4,900/month. Walk Score: 73 (very walkable).

Hospice notification failure: facility did not notify Department within 5 days of resident admission to hospice care (December 2025).

Recent inspections (October–December 2025) found zero deficiencies, indicating remediation progress. However, many other allegations investigated were unsubstantiated, and complaint-driven visit frequency remains exceptional.
