Meadow Valley is a 137-bed senior living community at 5143 North Long Lake Rd in Traverse City, Grand Traverse County, Michigan. The facility opened in 2024 under Oakleaf Village of Traverse City, LLC, offering independent living, assisted living, skilled nursing, and memory care services. Administrator David Phillips leads the operation. The facility holds a regular license, and respite care is available.
The inspection record documents a significant shift. In October 2024, an original licensing study found the newly constructed 54-bed facility in substantial compliance, with zero deficiencies noted. Eight months later, following the facility’s expansion to 137 beds, a June 3, 2025 renewal inspection identified two deficiencies related to food and dietary practices. Inspectors found documented meal census records were lacking and noted potential contamination risks involving uncovered food items and food storage practices. These concerns represent a meaningful change from the facility’s initial compliance record.
The community’s amenities include a fitness center and pickleball courts, supporting active recreation. Casual and fine dining options accommodate residents’ varied preferences and dietary needs. A movie theater provides entertainment and serves as a social gathering space. Meadow Valley benefits from exceptional walkability, with a walk score of 85 ranking it among Michigan’s top facilities for neighborhood accessibility. From October 2016 through January 2024, Fairview Grand documented 5 deficiencies, 38 percent below Michigan’s average of 8. The facility averaged 0.5 deficiencies annually, 38 percent fewer than the state average of 0.8 per year.
The original October 2016 licensing inspection found full compliance. A March 15, 2023 renewal inspection documented substantial compliance with zero deficiencies. However, the January 10, 2024 renewal inspection identified 5 deficiencies involving missing physician orders and incomplete service plan information for assistive devices, failure to update resident service plans annually, missing tuberculosis risk assessments for residents and employees, and inadequate staff training on reporting requirements, resident rights, safety and disaster planning, infectious disease containment, and medication administration. Census at the January 2024 renewal was 20 residents in a 35-bed capacity.
No complaint investigations appear in the facility’s history, and no fines or license suspensions are documented.

Harvest Pointe at Thornapple Manor is a 70-bed community at 2950 McKeown Road, Hastings, Barry County, MI, licensed as a Home for the Aged. Operated by the County of Barry and opened in 2023, the facility is very walkable with a walk score of 70, ranked 73rd of 702 Michigan communities, and situated at Hastings city center. The physical layout includes 50 residential units, with 20 configured for double occupancy.
The facility provides assisted living with rehabilitation services, short-term rehab, respite care, and memory care. Amenities include a full-service beauty salon, wellness program with on-site therapy and rehabilitation services, fitness center, free WiFi, personal linen service, concierge services, and ongoing life enrichment classes and opportunities. The facility does not accept Medicare or Medicaid and does not offer 24-hour staffing.
Initial licensing in February 2023 found two physical plant deficiencies: non-functioning exhaust ventilation in a common area toilet room and hot water temperature exceeding 120 degrees Fahrenheit in resident units. The facility addressed both promptly through corrective actions documented with submitted photos and videos.
The July 27, 2023 renewal inspection found zero deficiencies with substantial compliance. However, the October 9, 2024 renewal inspection documented one deficiency: medication administered to residents in applesauce or pudding without physician’s orders. At that inspection, census was 24 residents in the 70-bed capacity. Occupancy increased from 14 in July 2023 to 24 in October 2024.
No complaint investigations or enforcement actions are recorded.

Opened in November 2019, Cedarbrook of Rochester combines independent and assisted living with a dedicated 42-bed memory care unit at 790 Letica Drive, Rochester, Oakland County, MI. The facility’s 85 total beds rank it 176th among Michigan’s 437 senior living communities. CSL Rochester Master Operator, LLC operates the property. The neighborhood is moderately walkable with a walk score of 56 (ranked 170th of 702 Michigan communities), and the facility sits within walking distance of Rochester’s city center.
Cedarbrook offers respite care services and aspires to an upscale, welcoming environment. The original licensing documented both standard units and double-occupancy rooms across assisted living and memory care sections, with a design supporting safe, dignified senior living.
Five documented deficiencies since 2019 place this facility 38 percent below Michigan’s average deficiency count and 36 percent better on annual deficiency rates.
Most recently, a June 2025 complaint investigation found staff failed to respond to a resident’s repeated call light summons; the resident waited nearly 90 minutes on the toilet before receiving assistance, resulting in one substantiated violation.
An earlier April 2024 renewal showed substantial compliance with no deficiencies. April 2023 brought two substantiated violations: medication administration errors and untrained staff dispensing medications; though an April renewal that same month demonstrated full compliance.
July 2022 investigations revealed two violations: families were not included in significant care changes and family notification of a fall was delayed.
No fines or license suspensions are on record. The pattern across five years suggests persistent challenges in staff responsiveness and family communication protocols, without evidence of marked improvement or deterioration.
This is a moderately sized assisted living and memory care property with a history of compliance gaps centered on timely care response and next-of-kin notification.
South Lyon’s Cambrian Senior Living occupies 52365 W. 10 Mile Road, South Lyon, Oakland County, MI, in a highly walkable neighborhood (walk score 78, ranked 36th statewide). The 90-bed memory care facility is operated by Cambrian Of Lyon, LLC. Ranked 164th among Michigan’s 437 senior communities by capacity, the property advertises 24-hour staffing, farm-to-table dining, and specialized programming for dementia and Alzheimer’s residents.
Deficiency metrics place Cambrian 25 percent below state average, with 0.7 violations per year versus Michigan’s 0.9. Yet inspection documents reveal a troubling picture.
Opened February 2017 with zero deficiencies and full fire certification, the facility maintained that trajectory for six years. The gap widened in September 2023 when two violations emerged: service plans overdue for annual updates and PRN medications lacking proper administration instructions.
By July 2025, deficiencies had doubled to four, documenting medication administration errors with missed doses and failed documentation, uncovered garbage throughout the building, thermometers absent from resident food storage, and hazardous cleaning agents stored in locations accessible to residents with cognitive impairment.
No complaint investigations or enforcement penalties appear in available records. The inspection pattern: zero violations in opening, then emerging gaps around administrative compliance and safety systems, suggests either insufficient systemic controls or degrading oversight as census increased. Most troubling are the repeated medication-related findings (documented in 2023 and 2025) and the presence of chemically hazardous materials within reach of vulnerable memory care residents.
Occupancy climbed from 38 percent (34 residents, September 2023) to 63 percent (57 residents, July 2025).
