May 2023 documented failure to provide appropriate supervision during a behavioral incident, with unclear mechanisms for how a resident obtained scissors and why a fire extinguisher occupied the room. October 2023 showed one care staff member alone for four hours supervising 20 memory care residents. The same investigation noted a resident without a proper bed, only a recliner. October 2022 found incomplete medication administration documentation.

These deficiencies span multiple failure categories. Environmental safety, documentation discipline, staffing allocation, and supervisory rigor all showed gaps. The 2023 concentration of violations suggests systemic pressure or operational breakdown during that period rather than isolated incidents. The September 2025 clean inspection indicates the facility implemented corrections, but the breadth of prior failures deserves scrutiny before placement, particularly for residents requiring close oversight or medication precision.

Resthaven demonstrated compliance improvement by September 2025, yet families should ensure ongoing compliance given the 2023 scope of violations.

Original licensing in January 2023 documented substantial compliance with zero deficiencies. Within six months, July 2023 renewal identified three deficiencies. The facility was offering independent living services without department approval, a program statement issue rather than operational failure.

It failed to maintain a meal census record as required. A sink in the assisted living salon had very low water pressure.

October 2023 complaint investigation substantiated two deficiencies following allegations of medication impropriety and HIPAA breaches. Medical records were accessible in an unlocked copy room. Medications were not always administered per physician’s orders. Documentation gaps included failure to initial medication administration records and unclear duplicate medication orders.

Allegations about inadequate resident care, inadequate memory care staffing, and building maintenance issues were not substantiated based on investigation findings. May 2025 complaint investigation substantiated one deficiency for documentation errors in medication administration records and controlled substance logs. The investigation found no evidence that residents failed to receive prescribed narcotic medications, but several documentation errors existed. An allegation about resident skin breakdown from inadequate personal care was not substantiated.

The pattern across four inspections over 2.3 years reveals systemic documentation deficiencies and administrative lapses rather than evidence of withheld care. The medication documentation failures are material for compliance purposes. The HIPAA breach from unlocked medical records is serious and warrants questions about privacy protocols and corrective action. The facility’s compliance posture appears as a newer operation still calibrating administrative systems, particularly around medication recording and privacy controls, though care delivery allegations have not been substantiated by investigators.

Families should verify medication administration protocols, documentation safeguards, and HIPAA compliance improvements following the October 2023 and May 2025 substantiated deficiencies before placement.
