Staffing includes 220 employees (83 certified nursing assistants, 30 licensed practical nurses, 27 registered nurses) with round-the-clock coverage, supplemented by rehabilitation, dietary, and social work personnel.
From June 2022 through December 2025, the facility received 16 state inspections that yielded 49 health citations, including nine coded as critical. This represents 329% more deficiencies annually than the Texas average of 3.5 per year. Federal penalties of $211,000 were imposed across three separate citations in 2023 and 2024 for noncompliance with care and safety standards. The CMS overall rating stands at 1 star; health inspection scoring shows 65.4% below state average; quality measures trail Texas homes by 24.1%.
