The Community Care Licensing Division investigated two separate complaints involving the same 740‑type facility, ATRIA HILLCREST, during 2021‑2022. In the first inquiry, an unannounced visit was prompted by a concern that staff had failed to provide appropriate care to Resident #1. The evaluator met with Executive Director Sarah Dodd, interviewed the resident, the resident’s responsible party, staff, and reviewed documentation. Residents generally reported satisfaction with staff professionalism and respectful boundaries. Staff denied any lapses in care. Because neither the resident nor staff could identify specific incidents that substantiated the claim, the complaint was marked **unsubstantiated** and no deficiencies were recorded.

The second investigation encompassed a broader set of allegations—ranging from inappropriate exposure, physical harm, privacy violations, to neglect of fall prevention and timely response to call buttons. Multiple unannounced visits and extensive interviews with staff, residents, and a med‑tech, as well as record reviews, were conducted. In each allegation, the evidence gathered—including conflicting resident statements, police reports, and staff testimonies—proved insufficient to confirm wrongdoing. For instance, while one resident claimed staff had hit them, police investigations found no credible evidence of abuse. Likewise, despite claims of multiple falls, staff logs and fall‑risk assessments indicated the resident had received routine monitoring and that falls had diminished after intervention. Staff described following protocol for entering apartments, responding promptly to pendants, and ensuring privacy. Because none of the allegations could be substantiated, the report concluded with the same outcome as the first: **unsubstantiated** and without cited deficiencies.

Overall, both investigations reveal a pattern of thorough, unannounced assessments that consistently found no clear evidence of non‑compliance or neglect. While residents and staff expressed confidence in the facility’s operations, the complaints highlighted the importance of ongoing monitoring and documentation to maintain safety and care standards. The Department’s findings underscore that, even when serious allegations are raised, rigorous evidence collection is essential to determine whether regulatory standards have been violated.
