- BROOKDALE MIDWESTERN underwent a comprehensive inspection on August 13, 2024, resulting in five state standard violations. Of these, four were classified as Health Code deficiencies, including failures to train staff in reporting conditions and resident rights prior to job responsibilities, as well as inadequate background checks for new hires. The remaining violation was categorized under the Life Safety Code, specifically regarding heating, ventilating, and air-conditioning equipment not meeting referenced codes and standards. This issue had been previously identified on March 15, 2022, with a correction date of April 30, 2022. No Health Code deficiencies were listed in addition to these four.
