Inspection Report Summary for Atria Encino Terrace

ENCINO Terrace Senior Living, a 740‑type community care facility in Encino, California, maintained a consistent record of safety and compliance over the period covered by the California Department of Social Services (CCLD) reports. In a fall‑incident review dated 8 / 27 / 2021, a resident’s fall on 8 / 1 / 2021 was initially not reported to the agency because the staff deemed it unremarkable. The incident was later recorded, and the Licensing Program Analyst (LPA) instructed the facility to report all falls to CCL. No deficiencies were cited, and the LPA noted that the fall‑reporting protocol had been updated in response.

Infection‑control inspections conducted in January, October, and November 2022 found the facility in good order. The LPA observed comprehensive screening at the single entrance, required signage, adequate personal protective equipment, and proper hand‑washing and mask use. The facility’s fire safety systems, medication storage, linens, and food preparation areas met regulatory standards, and no health or safety hazards were identified during any of the visits. Census numbers ranged from 41 to 64 residents during the year, well within the 85‑bed capacity, and staffing practices continued to meet licensing requirements.

The October 2022 case‑management visit addressed a resident’s complaint of “rough” assistance during showering. The resident and executive director confirmed the incident was not sexual in nature; the caregiver ceased shower assistance and the facility instituted additional staff training on resident care. A police report was filed but closed without charges. The LPA reported no new hazards and noted that the incident had prompted enhanced training and procedural review. Across all visits, the facility demonstrated adherence to safety protocols and a willingness to correct procedural gaps, resulting in no cited deficiencies and maintaining a positive compliance record.
