The December 9, 2024 complaint investigation is the critical placement consideration. State surveyors documented substantiated verbal abuse with findings of “Actual Harm.” Residents reported staff used profanity and disrespectful language when responding to care needs, with one resident describing being called a “fucking bitch” and ordered to “shut the fuck up” while speaking to family on the phone. Four residents independently reported similar experiences on evening and night shifts, describing how staff language made them feel worthless, less than human, and upset.
